Appointments:
Clinic visits are scheduled on an appointment
basis. Our goal is to serve each patient efficiently and without
excessive waiting. Therefore, the receptionist may ask for basic
information regarding the purpose of your visit, in order to reserve the
necessary time required to meet your healthcare needs. If for any
reason you are unable to keep your appointment, please call 432-4587 as
soon as possible to cancel your appointment. At that time, the
receptionist will assist in re-scheduling your appointment if necessary.
Hours of Operation:
Monday through Friday, 8:30 a.m. to 5:00 p.m. with
extended hours on Monday and Thursday until 6:30 p.m.
Fees/Billing:
Fees for professional services, laboratory, x-ray and
other special services are based on the nature and extent of the
services rendered. All co-payments are due and payable at the time
of service. Medicaid recipients are required to show their cards
at each visit.
If you are receiving care for a work-related injury,
please notify the receptionist when you arrive for your
appointment. We will need to obtain employer information for
billing purposes.
Billing statements are sent out once a month.
All charges are due and payable within 30 days. If you have any
questions regarding your account please feel free to call. We'll
be happy to answer your questions.

Insurance:
It is the patient's responsibility to provide our
business office with accurate insurance policy information. We
will submit claims to your insurance company as a courtesy.
However, the account balance is ultimately the patient's
responsibility. For those without insurance, payment plans are
available.
All providers at the Milbank Medical Center
participate in Medicare Part B. Claims are sent by the clinic, and
Medicare sends payments directly to the clinic. The clinic also
files claims for supplemental Medicare insurance. Patients with
supplemental insurance need to provide policy information to the
business office so that claims can be sent.

any questions you have regarding insurance processing.
